When the director of Manaka Hospital dies, an election is held between two doctors who have been recommended for the position of new director. Dr. Shiitake has the same 20 years of service as the other two candidates, but because he is a low-key person who is treated like an assistant, no one recommended him. Meanwhile, a collapse occurs at a construction site in the city. Black Jack happens to be nearby, and Shiitake, who is on his way home, provides first aid to the injured at the scene. Black Jack is surprised to learn that Shiitake, who has advanced medical skills, is just a regular member of the medical staff at Manaka Hospital.

Hisao tries to skip school by faking an illness, but his mother finds out. Hisao doesn't want to go because he doesn't want to be scolded by his homeroom teacher, Mr. Muramasa, for not being able to do math. The next morning, on his way to school, he is hit by a motorcycle, thinking that if Hisao gets sick or injured, he won't have to go. Muramasa is shocked to hear that Hisao is in such a critical condition that both of his legs will need to be amputated. Ichinoseki Kazuo, a high school student, is a popular gymnast who is set to compete in the Inter-High School Championships. After school, Kazuo practices on the rings and feels pain in his left arm, but he endures it and continues practicing. His childhood friend, Furukawa Toshie, notices something strange about Kazuo's left arm and calls his mother, who then takes Kazuo to Tezuka Clinic for a checkup.
