Black Jack and Pinoko are traveling in Nagano when they see Tsuyuko with bulging eyes and a swollen throat on the bus. As Tsuyuko's mother comforts her in pain, Pinoko suddenly develops a stomachache. Black Jack decides to have Pinoko examined at Furuwa Hospital in Yamaya Village, where the mother and daughter are heading. Black Jack and his friends follow the mother and daughter to Furuwa Hospital, which is tucked away deep in the mountains. After prescribing medicine for Pinoko, Dr. Furuwa examines Tsuyuko. Furuwa tries to cure her with medicine, but Black Jack advises that surgery would be better. Tsuyuko has Graves' disease...
