Black Jack is performing a difficult operation on a thoracic aortic aneurysm at a hospital, while a representative from the World Medical Associations Federation watches him. Meanwhile, Pinoko, who is at home alone, discovers the thief Genkichi. After hearing Genkichi's unfortunate story, Pinoko promises to bring him the money when she has it. The next morning, Black Jack receives a call from the World Medical Associations Federation. They have learned that the talented Black Jack is an unlicensed doctor, and will be negotiating with the Japan Medical Association to have him issued a medical license. Meanwhile, Pinoko is visiting Genkichi's house with the pocket money she borrowed from Black Jack...
