When the director of Manaka Hospital dies, an election is held between two doctors who have been recommended for the position of new director. Dr. Shiitake has the same 20 years of service as the other two candidates, but because he is a low-key person who is treated like an assistant, no one recommended him. Meanwhile, a collapse occurs at a construction site in the city. Black Jack happens to be nearby, and Shiitake, who is on his way home, provides first aid to the injured at the scene. Black Jack is surprised to learn that Shiitake, who has advanced medical skills, is just a regular member of the medical staff at Manaka Hospital.
