One of the few things that both Donald trump and Hilary Clinton seemed to agree on is that high out-of-pocket spending, specifically as it relates to the Affordable Care Act, is a problem. One of Clinton's most popular health care proposals during her campaign was to reduce out of pocket spending to more "manageable" levels for many Americans. President-elect Trump sought to fix this problem by repealing the ACA and replacing it with something better.
Reducing out-of-pocket spending, however, will require some tradeoffs. No easy solution exists, but there are examples out there worthy of consideration. That's the topic of this week's Healthcare Triage.

The recent election has caused many to question whether significant changes are about to happen to Medicaid. Repeal of the Affordable Care Act would, of course, lead to the elimination of the Medicaid expansion, which could result in significant numbers of adults losing their coverage overnight.
But even without repeal, many Republican replacement proposals also result in significant changes to Medicaid, whether it be through funding, eligibility, or benefits. So, again, let's talk about what Medicaid does, and whether it's long term benefits are worth it. This is Healthcare Triage News.

Obamacare's marketplaces were supposed to give consumers choices of health plans from insurers that compete to keep premiums down. But fewer insurers are participating, and premiums are increasing sharply.
Fixing this problem will obviously be politically difficult with a Republican-controlled Congress that has vowed to "repeal and replace." President-elect Donald J. Trump has also said he wants to get rid of the ACA, although he amended that recently by saying he'd like to keep some elements. Replacing the law, without a Senate supermajority, would also be politically difficult.
From a policy standpoint, however, some solutions to problems facing the marketplaces are ones that Republicans have endorsed before: for Medicare. That's the topic of this week's Healthcare Triage.

The filibuster still poses some potential threats to their plans, but it's also within their means to abolish its widespread use in such a way that they could both repeal the Affordable Care Act and replace it with something of their own design.

Compounding this problem, many of the risk factors for heart disease seem "baked into the cake". Genetic factors can have a huge impact on people's risk, and those factors are almost always outside of one's control.
Because of this, there are some who feel a certain amount of powerlessness to prevent heart disease. Recent research contradicts this, though, and should give us all renewed hope. That's the topic of this week's Healthcare Triage.

I once received a lot of blowback for an Upshot article in which I showed (with evidence) that exercise is not the key to weight loss. Diet is. Many, many readers cannot wrap their head around the notion that adding physical activity, and therefore burning more calories, doesn't necessarily translate into results on the scale.
Well, here we go again because some of those folks also believe that fitness devices - Fitbit, Vivosmart, Apple Watch - must be helpful in losing weight. Unfortunately, evidence doesn't support this belief either. That's the topic of this week's Healthcare Triage.

Cystic fibrosis is an inherited disorder that affects the lungs, pancreas, intestines and other organs. A genetic mutation leads to secretory glands that don’t work well; lungs can get clogged with thick mucus; the pancreas can become plugged up; and the gut can fail to absorb enough nutrients.
It has no cure. Over the last few decades, though, we have developed medications, diets and treatments for depredations of the disease. Care has improved so much that people with cystic fibrosis are living on average into their 40s in the United States.
In Canada, however, they are living into their 50s. That’s the topic of this week’s Healthcare Triage.

Last week, Republicans in the Senate didn't quite manage to repeal and/or replace the Affordable Care Act. So, why exactly can't Mitch McConnell, Paul Ryan, and Donald Trump get Obamacare repealed. For one thing, Susan Collins and Lisa Murkowski were stalwart in their resistance to taking away people's insurance without a plan to replace it. And some late-night dramatics from John McCain put the bill over the top. Or, under the bottom, I guess. It didn't pass.

This week, we'll be looking at the history of guns in the US, and how gun legislation has changed over the last couple of hundred years, and how courts' interpretations of the second amendment have changed. In the next few weeks, we'll look at gun homicides, gun suicides, and firearm policy.

It's hard to research though, because of the drugs' legal status as Schedule I drugs. This is complicated, though. Breathalyzers for alcohol work on principles that don't necessarily work for pot. And they've been extensively researched, which is difficult to do with federal controls on marijuana. There may never be a breathalyzer for pot, but one thing's is for sure: eating your own feces won't beat a breathalyzer.

From 2009 onward, federal policy about sex education tended to favor an evidence based approach. If a sex education program worked, and was supported by rigorous evidence, the federal government would potentially back it. This was true of sex ed programs that taught students about STDs and contraceptives, and abstinence-based programs. As long as there was evidence for it, the program was eligible. In the last year, policy has dictated that the federal funding show a preference for abstinence only programs. And spoiler alert, the abstinence only programs don't work that well.
