The best way to prevent transmission of Ebola in the United States, or any country for that matter, is to identify and quarantine those with the disease as soon as possible. However, the first person diagnosed with Ebola in the US was, unfortunately, released after coming to an emergency department, even though he had symptoms indicative of the disease.
He was sent home on antibiotics. The inappropriate use of antibiotics is the subject of this week's Healthcare Triage.
This was based heavily on a piece I wrote for the Upshot, which you can go read here: http://www.nytimes.com/2014/10/21/upshot/on-an-antibiotic-you-may-be-getting-only-a-false-sense-of-security.html. All the refs and links are there.

I've found that of almost all of the topics I write about over at my blog, the one that stirs up the most controversy, the one that generates me the most hate, the most emails and the most tweets is e-cigarettes. Defenders of them are very passionate, and surprisingly organized. Those that dislike them are no less dedicated. But let's get past the rhetoric. E-cigarettes are the topic of this week's Healthcare Triage.

Last week we talked about sleep. We talked about how much the average person needs, and how much they get. We also talked about how you can't just rely on "averages" to determine how much you need. Sleep in a personal thing, and we all need different amounts.
But sleep is incredibly important. You have to do it. Not getting enough, or sleep deprivation, is a real, and bizarre thing. It's also the topic of this week's Healthcare Triage.

When we are judging the cost-effectiveness of a treatment or intervention, we're really asking how much bang for the buck we're getting for our healthcare spending. That can be relatively easy when we're talking about life and death. But how do we measure improvements in quality? The most widely used method is through the use of utility values, and we'll show you how we calculate those in this week's Healthcare Triage.

In previous episodes of Healthcare Triage, we've discussed how people often misunderstand risk and how it affects them. We've also talked about how you can calculate the explicit metrics of numbers needed to treat and harm. But people rarely use them. In spite of the existence of metrics to help patients appreciate benefits and harms, a new systematic review suggests that our expectations are not consistent with the facts.
Most patients overestimate the benefits of medical treatments, and underestimate the harms; because of that, they use more care. That's the topic of this week's Healthcare Triage.
This piece was based largely on a column Aaron and Austin Frakt wrote for the Upshot at the NYT. References can be found there: http://www.nytimes.com/2015/03/03/upshot/if-patients-only-knew-when-more-information-means-less-treatment.html

Whenever I give a talk on the sorry state of the US health care system, someone asks me what we should do. My first comment is always something along the lines of "if we knew what to do, we'd have already done it". But if I'm pushed, I will usually note that the best first step would be for us to stop doing things that don't work.
There's an organization dedicated to that. It's called Choosing Wisely. It's also the topic of this week's Healthcare Triage.

1:34 - There's so much dispute over multivitamins for adults & kids. Some say they are worthless, others say they are essential. I'm pregnant & there seems to be NO dispute over me taking a prenatal, why?
4:14 - I have recently been told by a woman that there are all these studies about bras causing breast cancer. have you heard of any? Is she just a fearmonger? Sounds awfully suspicious to me.
5:15 - What are your thoughts on breastfeeding? Are the benefits overhyped? Are the recommendations (6 months exclusive) practical?
7:09 - what do you think about gene therapy?
9:12 - is the silverchloride in anti-perspirants harmful? If yes, are there alternatives?

13:18 - Are Doctors of Ostheopathy (DO) as good as regular doctors (MD)?
13:58 - Our pediatrician has been very adamant about our 14 month old drinking whole milk due to high fat requirements for the developing brain. Is this legitimate? At what point is milk no longer beneficial?
15:43 - What is up with food dyes and adhd? Hoax or just circumstantial? Or is there actually a causational link?
17:04 - Ok, seriously though... why is the lens of the microscope in your "To the Research" poster shaped like a condom?
17:43 - What kind of evidence/studies are there showing support for the idea that pregnant women shouldn't consume certain things such as sushi or coffee?
19:33 - How good is the research on vegan lifestyles and their health benefits?
21:00 - Any perspective of genetic counseling and the importance of proactive genetic testing?

I spend a lot of time writing and talking about what isn't true. Over the past few months, we've had lots of episodes talking about how many nutrition recommendations aren't supported by science. I've argued that what many people are telling you may be inaccurate. In response, lots of you have asked me what nutrition recommendations should say.
The truth is that it's much easier to tell you what not to do, than to tell you what to do. But we don't avoid the hard questions. Recently I shared my nutrition recommendations over at the Upshot at the New York Times, and they were surprisingly popular. So I'm going to share them with you, here on Healthcare Triage

We've covered test characteristics like sensitivity and specificity before, and we've even covered how you can use those values to make decisions about what results mean about your health. But sensitivity and specificity don't have a meaning that people usually understand.
What people want to know are other values, what we might call the positive predictive and negative predictive values. But there are problems with those metrics. They're the topic of this week's Healthcare Triage.

We seem to have no problem penalizing patients for being unhealthy (see our episode on wellness programs). But we rarely consider paying patients to be healthier. Why is that? The NEJM has a study to change your mind.

Why do doctors get sued? How can malpractice suits be avoided? It turns out, the answer may be simple. Defensive medicine refers to the idea that doctors are forced to order extra tests, perform extra procedures, or push for more office visits because they think that without them, they're at greater risk for being sued. This is in spite of the fact that studies don't support the notion that this extra care actually does reduce their risk.
What might help physicians to get sued less often would be for them to get along better with their patients. Or at least, they could become better communicators. That's the topic of this week's Healthcare Triage.
This was based on a piece Aaron wrote for the NYT. Links to further reading can be found there: http://www.nytimes.com/2015/06/02/upshot/to-be-sued-less-doctors-should-talk-to-patients-more.html

What are the benefits of exercise? Is exercise all that good for you? Yes, yes, and yes. Studies overwhelmingly indicate that exercise has a LOT of benefits. I wrote a piece on exercise and weight for The New York Times a couple months ago, and I'm going to talk about that next week. But a lot of people took that column to mean that exercise isn't important or helpful. That's so wrong that I want to address that first. Exercise, and the good it can do you, is the topic of this week's Healthcare Triage.

What is alternative medicine? Does alternative medicine work? The University of Toronto recently stirred up a bit of controversy by uncritically offering a class on "Alternative Medicine." When people looked at the coursework, which included many studies debunked by this channel, some people started to freak out. The issue highlights a much larger problem: No one is sure how best to teach that subject.
The dichotomy, however, between alternative and traditional medicine, or between Eastern and Western medicine, is a false one. We would be much better off if we could reframe the issue. That's the topic of this week's Healthcare Triage.
This episode was based on a post I wrote for the Upshot. Links to sources and more reading can be found there: http://www.nytimes.com/2015/08/11/upshot/labels-like-alternative-medicine-dont-matter-the-science-does.html

How much water should you drink every day? Not as much as you've been told. This summer, like many summers before it, has seen a rash of articles warning us not only that is dehydration is dangerous, but that it is also ubiquitous. Real dehydration, when your body has lost a significant amount of water because of illness, excessive exercise or sweating, or an inability to drink, is a serious issue. But people with clinical dehydration almost always have symptoms of some sort.
These reports are worried about something completely different. They fear that otherwise healthy adults and children are walking around dehydrated, so many that dehydration has reached epidemic proportions. Under scrutiny, however, this assertion fails to hold water. That's the topic of this week's Healthcare Triage.

Let's start at the beginning. What's in a name? That which we call a drug. By any other name would be so . . . effective? Brand name and generic drugs: are there any differences? Who regulates them? And what's with all the names anyway? That's the topic of today's HealthCare Triage.

When people hear about vaccine denial, they most often think about parents who are refusing to vaccinate their children. But there's another type of vaccine refusal, and it's important that we not ignore that, too. Doctors sometimes promote the use of some vaccines with less enthusiasm than others. Sometimes, they don't talk about them at all.
This occurs most often with the herpes simplex virus, or HPV, vaccine. Our suboptimal immunization rates with this vaccine, and the behaviors of the physicians who might be contributing to that, have consequences. That's the topic of this week's Healthcare Triage.
This is based largely off a post Aaron wrote for the Upshot. Links to further reading can be found there: http://www.nytimes.com/2015/11/10/upshot/hpv-vaccine-myth-good-talks-needed.html

You guys love the microbiome. You're not alone. The sales of products containing yeasts and bacteria that will colonize your gut and make you stronger and better are legion. I recently met a researcher who did actual microbiome work, and he didn't seem nearly as convinced as the public. But do probiotics work? That's the topic of this week's Healthcare Triage.

1:52 - Several studies in the last few years have shown Ketamine are effective in short term management of depression. Do you see this and other treatments using illegal drugs, MDMA for PTSD, gaining use?
3:39 - Barring personality disorders, is there research on if/how personality affects psychological therapy? Does therapy work better for some people or is it just a matter of finding the right doctor?
5:11 - Should every breast cancer detected in the screening program be treated? I read a 2014 BJM paper where it said that some disappear or stay that way and don't grow.
8:16 - Are there other options besides Imuran as far as immunosuppressants go to treat Ulcerative Colitis?
9:02 - Is arsenic in rice something to be concerned about? It sounded like nonsense to me at first, but then I looked it up and apparently there's a Consumer Report on it and the FDA's been looking into it.
10:24 - I've been exercising for the last five months, three times a week 30 minutes on the elliptical; I would like to increase to four, five times a week, on your experience, would this be optional? Male, 34
11:06 - What do you think of health education in the US? What changes would you make especially in public school health education?
12:43 - My niece (2 yrs) has had a pretty consistent stream of snot from her nose over the last several weeks, and her mom medicates her for it. My wife says kids will just be snotty, drugs or no. Who's right?
14:39 - Are there differences in health outcomes and mortality between for profit, non profit, and government/military run hospitals?
15:23 - Why do pre-med students need to take physics?
17:27 - How do you respectfully tell a doctor that you don't agree with his/her diagnosis? Is there a way that YOU would be okay with being told I wanted a second opinion?
19:20 - Are there any stats on student doctor outcomes/complications as opposed to experienced doctors? Any advice as to getting into medical school?
20:38 - Do
