Health professionals work closely with families that are badly in need of advice as to how they might transform their lifestyles and ensure their future wellbeing. In the first edition, a doctor comes up with an eight-week health plan for the Mahony family, whose members consume too much alcohol and junk food in relation to how little exercise they do.

Doctors devise an eight-week health plan for the Porter family from Naas, Co Kildare, whose members realise they have a good deal of work to do to improve their future wellbeing. Christy, who is 68 years old, used to be active but is a heavy smoker, while his wife Margaret is overweight and takes a large concoction of pills daily, and his daughter Gwen has a fondness for takeaway food.

Brigid Broekhoven, a 50-year-old former alcoholic living in Blackrock, Dublin, becomes concerned about her increase in weight, while her 20-year-old daughter Lizzy is worried about her irritable bowel syndrome. The pair seek expert guidance in improving their lifestyle, but when a CT scan provides evidence that Brigid has a possibly cancerous growth on her kidney, the check-up proves to be more than timely. While her mother heads for surgery, Lizzy is informed that her family's health history means she must reduce her high cholesterol by revising her eating habits and increasing her fitness level.

Doctors travel to the home of Sinead Mills and Jason Mooney in Tallaght, south Dublin. Both long-term smokers, the couple admit to lighting up in the house, but they are also concerned that the diet of their two sons - ages 12 and 13 - is not up to scratch. As well as that, the entire family could do with some more exercise according to the visiting medics, who remain in their house to oversee the changes that could have a positive effect on the household's health.

Doctors travel to the home of students Conor, Padraig, Brian and Paddy, who share a house in Athlone. Conor has a family history of heart problems, Padraig likes a drink, and Brian is addicted to sugar, so the medics come up with ways to transform the household's diet, and remain in the property to oversee changes that could have a positive effect on their long-term health.

Doctors travel to the home of engaged couple Eileen and Peter, who live together in Balbriggan, Fingal. Eileen has a condition called fibromyalgia, which causes her widespread pain and fatigue, while Peter admits that, as well as being underweight, is also a smoker and has had alcohol problems. The medics come up with ways to transform the household's diet, and remain in the property to oversee changes that could have a positive effect on their long-term health. Last in the series.
