Barış, who learns that Gizem is pregnant and will become a father, has to make a decision about cancer treatment. The opening of the Penitentiary Department deeply affects all the doctors in the hospital. Kemal Kuyucu, the dean who came to the opening, has a surprise for Barış! Things get complicated when Güldal Sipahi, the famous name of the society, who comes to the hospital to donate, encounters patients from the Penitentiary Institution. While Derin and Aras try to mend the relationship between them, Aras experiences a great shock. On the other hand, Ali Haydar and Andaç try to treat a very rare patient with Cotard syndrome.
