A woman who has selflessly set up a charity to help one person in need a week is reunited with some of the people she has helped; a man who was inspired to set up the first homeless football league in Wales is recognised for all his dedication to helping countless people on the fringes of society; and an inspirational and extraordinary teacher who has changed the lives and future of hundreds of children gets a well deserved thank you, and receives a lovely surprise for the children at her school.
