In Texas, a massive explosion in a fertilizer plant rocks the state, and an unlucky construction worker is forced to choose between a potentially lethal leap or a fiery death. In Utah, a base jump goes wrong, a botched rescue at a frozen lake in Wyoming leaves almost a dozen people floundering in icy water, and a homemade hot-air balloon lands on a huge pile of fireworks just meters from a crowd of thousands in Burma.
