Carly is critically injured in an elevator accident. Marcus performs emergency surgery while Bo fears for her life, sensing her flatline moment. After stopping the bleeding, Marcus warns the next 72 hours are critical. Bo becomes convinced Victor orchestrated the accident to kill him, not Carly. Caroline initially disbelieves him but confronts Victor and recognizes the truth in his eyes, devastated that she's spent years defending a monster. Jennifer finds comfort in Jack's support at the hospital. Meanwhile, on the island, the two Romans clash and one shoots the other in the leg. Marlena hears a voice calling "Doc" and encounters one Roman, who carries her away to a shack after she faints at the sight of him. The other Roman lies wounded in the mud. Elsewhere, Cal attempts breaking into Kim's house while Shane and Kim have a playful paint-roller fight inside, interrupted by Kayla's arrival with news of Carly's accident.
